APPLICATION FOR A NEW PASSPORT , REPLACEMENT OF A LOST OR DAMAGED PASSPORT
1. PROCEDURE OF APPLYING FOR A NEW PASSPORT:
A) ADULT APPLICANTS
i) A duly completed form ‘A’
FORM ‘A’ Download here
ii) A duly completed form ‘N’
FORM ‘N’ Download here
iii) A bar code form (obtainable from the Embassy)
iv) Two (2) recent biometric passport sized photographs with a white background (35 x 45 mm).
IMPORTANT: These must be taken full face without hat, hair extensions, earrings and a necklace. One of which must be endorsed by a recommender.
v) A photocopy of Birth Certificate or Affidavit of Birth

vi) A photocopy of the National Registration Card (NRC)
vii) Copy of Marriage Certificate for married applicants
B) APPLICANTS UNDER 16 YEARS
i) A duly completed form ‘B’

iii) A bar code form (obtainable from the Embassy)
iv) Two(2) biometric recent passport sized photographs, one of which must be endorsed by a recommender.
v) A photocopy of Birth Certificate or Affidavit of Birth

vi) A letter of consent from either Parent
vii) Copy of the parent’s NRC and passport copies
viii) Copies of Birth Certificates of children or Affidavits in support of children to be endorsed/included on the passport
2. PROCEDURE FOR REPLACEMENT OF LOST OR DAMAGED PASSPORT:
i) A duly completed form ‘A’ and ‘N’ for adults and ‘B’ and ‘M’ for those below 16 years
ii) Two (2) recent passport sized photographs, one of which must be endorsed by the recommender
iii) A duly completed Lost Passport Description Form ‘L’
iv) A lost passport report from the Police
v) Lost Passport – Affidavit
vi) A photocopy of National Registration Card (NRC), Birth Certificate or Affidavit of Birth
vii) A bar code Form (obtainable from the Embassy)
3. PROCEDURE FOR CHANGE OF NAMES:
A change of the name other than by marriage or adoption must be substantiated by the production of evidence showing that , A bonafide change has been made for all purposes.
i) A photocopy of any of the following: Marriage Certificate, Divorce Certificate or Deed Poll

4. PROCEDURE TO ACQUIRE TRAVEL DOCUMENTS OF IDENTITY:
Travel Documents are issued in cases of emergency travel only and are valid not exceeding six (6) months of the one way validity clause to persons returning to Zambia.
i) Copies of the National Registration Card (NRC)
ii) A duly completed Travel Document of Identity Form (obtainable at the Embassy)
iii) FEES: Twenty Euros ( € 20.00 )
